1) Which of the following arrangements do you use to measure On-Time-Delivery(OTD)?
  a) Same window for all suppliers
  b) Different windows for different suppliers. (Select all that apply):
     i. One window for local suppliers: delivery the day requested
     ii. One window for international suppliers: 2 days before - 2 days after window
     iii. One window for milk runs: delivery within hours on the day requested
     iv. Windows by type of supplier: A suppliers, B suppliers, C suppliers
     v. Others (please specify):______________________

2) Do you include arrears in the measurement? If so, how do you include/measure the arrears?

3) For the suppliers that you expect to have a Min-Max inventory, how do you measure OTD?
  a) Measure OTD in similar manner as for any other supplier 
  b) Measure the amount of times within Min-Max (If so, how do you measure this?). 
  c) Measure both OTD and amount of times within Min-Max
